Abstract
Conflict of interest: none declared. An 81‐year‐old man presented with a 2‐year history of waxing and waning nodules on his right lower leg. The lesions had initially appeared 6 months after he had been pricked on the right lateral knee by a rose thorn. The nodules, which appeared in crops and were at different stages of evolution, resolved spontaneously within a few months and then recurred. However, the nodule on his right knee persisted throughout the course of the disease. On physical examination, multiple, erythematous, necrotic nodules were seen in a sporotrichoid pattern on the posterior calf (Fig. 1). Two of the nodules were purulent. There was no evidence of hepatosplenomegaly or lymphadenopathy. An 8‐mm punch biopsy sample was obtained from a nodule on the right knee. ... On histological examination of the nodule, a dense, large, lymphoid‐cell infiltrate was seen, occupying and expanding the entire reticular dermis without involving the overlying epidermis. The lymphoid cells displayed moderate pale or eosinophilic cytoplasm and large oval to irregular vesicular nuclei, some kidney‐shaped, and containing one or more prominent nucleoli (Fig. 2). There was brisk mitosis. Immunochemical staining showed strong and diffusely positive staining in the lymphocytes for CD30 (Ki‐1), with a dot and membrane staining pattern, (Fig. 3). CD43 was weakly positive, with variable loss of the pan T‐cell antigens CD3, CD7 and CD5. Staining for anaplastic lymphoma kinase (ALK)1, epidermal membrane antigen (EMA), CD20, CD45, CD79, and CKAE1/AE3 were negative.
