La trypanosomiase humaine africaine ou maladie du sommeil est une maladie parasitaire d'Afrique sub-saharienne transmise aux humains par la mouche tsé-tsé. Il existe deux types de maladie du sommeil, qui sont causées par deux sous espèces de parasites : Trypanosoma brucei gambiense, que l'on retrouve en Afrique occidentale et centrale, et Trypanosoma brucei rhodesiense, que l'on retrouve en Afrique orientale et en Afrique australe. La forme la plus répandue est due à Trypanosoma brucei gambiense. La maladie suit deux stades. Chancre, trypanides, adénopathies, fièvre, céphalées, prurit et oedèmes au cours du premier stade ou stade lymphatico -sanguin. Le second stade, dit méningo-encéphalitique, se produit lorsque le parasite franchit la barrière hémato-encéphalique et envahit le système nerveux central. Les patients présentent alors des signes neurologiques: réflexes anormaux, troubles moteurs, troubles sensoriels et mouvements anormaux. La maladie progresse vers des troubles du rythme veille-sommeil, la détérioration mentale et, finalement, le coma. En l'absence de traitement, la maladie est mortelle. L'homme est le réservoir du parasite Trypanosoma brucei gambiense. On peut donc lutter contre la trypanosomiase humaine africaine et contrôler sa transmission en procédant au dépistage de masse de la population et au traitement de tous les patients infectés. Le dépistage de masse nécessite des équipes mobiles qui visitent tous les villages de la zone touchée et qui effectuent un dépistage sur une proportion aussi élevée que possible de la population. La lutte antivectorielle est une autre composante importante de la lutte contre cette maladie.
