ALLHAT, étude clinique d’une ampleur inégalée par l’importance des patients inclus et la durée d’observation des sujets, propose l’utilisation en première intention des diurétiques thiazidiques dans le traitement de l’hypertension artérielle. Si cette étude soulève quelques critiques, en particulier méthodologiques (choix du diurétique, administration croisée de plusieurs médicaments pouvant atténuer les différences entre les groupes), le caractère provocateur de ses conclusions conduit à une réhabilitation de cette classe d’agents anti-hypertenseurs dont les effets extra-rénaux peuvent être bénéfiques à condition de tenir compte de leurs propriétés pharmacodynamiques.
