Patients : Pour participer a l’etude, les patients devaient avoir un diagnostic d’hypertension arterielle legere de grade 1 selon les criteres de l’Organisation mondiale de la Sante (140-159/90-99 mm Hg) et ne pas recevoir de traitement antihypertenseur. Ces patients etaient suivis a l’unite d’hypertension et de risque vasculaire de l’Hospital Clinico Universitario de Santiago de Compostela, en Espagne. Les criteres d’exclusion a l’etude etaient : quarts de travail en alternance, hypertension arterielle moderee ou severe (tension arterielle systolique > 159 mm Hg, tension arterielle diastolique > 99 mm Hg), hypertension arterielle secondaire, probleme cardiovasculaire autre que l’hypertension arterielle, apnee du sommeil obstructive, contre-indication a l’utilisation de l’aspirine. Interventions : Les patients admissibles a l’etude etaient assignes de facon aleatoire a l’un des trois groupes suivants dans un rapport 2/1/1 : mesures non pharmacologiques (MNP), MNP + aspirine 100 mg po DIE le matin, MNP + aspirine 100 mg po DIE au coucher. La dose de 100 mg correspond a l’aspirine a faible dose disponible en Espagne au moment de l’etude. Les MNP etaient : restriction sodique, remise d’information sur l’approche dietetique recommandee pour diminuer la tension arterielle, limitation de la consommation d’alcool et exercice physique aerobique regulier. L’observance aux mesures non pharmacologiques etait estimee a l’aide de tests sanguins (triglycerides, cholesterol), d’une collecte urinaire de 24 heures (sodium, potassium) et de la variation du poids, du tour de taille et de hanche ainsi que de l’indice de masse corporelle. L’observance au traitement pharmacologique etait evaluee au moyen du decompte des comprimes et d’entrevues. La tension arterielle etait mesuree a l’aide d’un tensiometre ambulatoire pendant 48 heures avant l’intervention et 3 mois apres le debut de l’intervention. L’appareil enregistrait la tension arterielle systolique, la tension arterielle diastolique, la tension arterielle moyenne et la frequence cardiaque aux 20 minutes le jour (7 h a 23 h) et aux 30 minutes la nuit. La meme personne expliquait le fonctionnement de l’appareil a un patient a ces deux reprises. Les patients ne pouvaient pas lire les valeurs des parametres mesures par l’appareil. La tension arterielle etait aussi evaluee a ces deux moments par le chercheur (6 mesures apres que le patient a ete assis pendant au moins 5 minutes). Le meme chercheur etait assigne a un meme patient lors de ces deux visites.
