Abstract
Un traitement intensif du diabete par insuline reduit les complications a long terme, mais augmente l'incidence des episodes hypoglycemiques. Seule une transplantation de pancreas ou d'ilots de Langerhans peut permettre l'arret du traitement par insuline, un reglage glycemique adequat et une prevention des complications. La transplantation de pancreas doit etre envisagee chez tout patient diabetique en attente d'un greffon renal. Une serie canadienne a demontre 100% d'insulino-independance quinze mois apres transplantation d'ilots seuls (douze patients consecutifs). Ces resultats prometteurs elargissent les indications a la greffe d'ilots : rein-ilots simultanes, ilots apres rein et ilots seuls. Cet article decrit l'experience genevoise en allotransplantation d'ilots, discute les resultats canadiens et redefinit les indications a la transplantation chez le patient diabetique.
