Abstract
Resume Les divisions labiomaxillaires constituent une dysmorphose congenitale commune dont la prise en charge therapeutique, qui s’etend de la naissance a la fin de l’adolescence, est multidisciplinaire. Les calendriers therapeutiques et les techniques chirurgicales utilisees varient d’un centre a l’autre, tout comme le recours au traitement orthopedique prechirurgical. Aujourd’hui, les plaques palatines sont preconisees par certains, decriees par d’autres. Il en existe de multiples sortes : fixes ou amovibles, actives ou passives, combinees ou non a un appareil extraoral. Apres un rappel des principales classifications des divisions labiomaxillaires, differents types de plaques palatines et leur technique de realisation sont decrits. Le traitement orthopedique preoperatoire est certes tres controverse, mais les effets benefiques attendus a court, moyen et long termes ne sont pas identiques selon les equipes soignantes. Il reste difficile de faire la part des choses entre l’action conjuguee de la chirurgie, du traitement orthopedique, du degre des dysfonctions, du polymorphisme initial des fentes, des potentialites de croissance variables d’un individu a l’autre. Etant donne la variabilite des protocoles therapeutiques d’un centre a l’autre, le role de ces traitements orthopediques prechirurgicaux reste tres complexe a evaluer.
