Expanded Pure Endoscopic Endonasal Approach for Clival Lesions

Abstract
Widespread use of the endoscope in transsphenoidal surgery has recently contributed to the extension of approaches beyond the tuberculum sellae for the management of suprasellar, anterior skull base or clival lesions. Based on our experience with the standard purely endoscopic approach to the pituitary fossa, we evolved the use of the expanded pure endoscopic endonasal approach to address clival lesions. Because of their midline location, clival neoplasms are suitable indications for this minimally invasive technique. However, those with significant lateral extension cannot be dealt with by this technique as a sole approach and open transcranial procedures need to be combined with the endoscopic approach. The technique described below incorporates technological advances in endoscopic endonasal surgery, frameless stereotactic guidance and novel skull base reconstruction methods. Our results in terms of tumor removal rate, complications and survival are similar to those reported in previous series. The minimally invasive nature of this approach with comparable results makes the technique very attractive and probably the procedure of choice for selected clival lesions.
