Background: Endoscopic endonasal approaches (EEAs) have been applied for pathologies across the entire anterior skull base. Olfactory groove meningiomas are tumors of the anterior base that present significant challenges. Objectives: To evaluate the surgical techniques, challenges, advantages and disadvantages of EEA for olfactory groove meningiomas. Methods: We present our concepts of EEA for addressing these tumors by thoroughly discussing our surgical techniques, presenting the advantages of the approach such as early devascularization and complete lack of brain retraction, and reviewing outcomes, especially complications. Conclusions: Olfactory groove meningiomas can be addressed safely and effectively via EEA, with the conceptual advantage of accessing the tumor surface directly.
