Over a 10-year interval, major advances in endoscopic technique as well as instrumentation to facilitate the endoscopic endonasal approach (EEA) to the skull base have been reported. The authors describe the evolution of and resulting methods for reconstruction of the skull base following EEA using multilayer, free tissue grafts to the more recent adoption of the vascularized nasoseptal flap. Other options such as the pedicled inferior turbinate flap and temporoparietal fascial flaps are also discussed. Technical aspects of flap harvest and application are discussed as well as risk factors for CSF leak. The high-risk CSF-leak patient is identified, and methods to reduce CSF leak to less than 5%are described.
