Effects of dornase alfa and hypertonic saline on gas mixing in lung periphery in children with cystic fibrosis

Abstract
Background : Inhaled dornase alfa (DA) and hypertonic saline (HS) were shown to improve lung function as quantified by the lung clearance index, a measure of overall ventilation inhomogeneity (VI) from multiple-breath washout (MBW), in children with Cystic Fibrosis (CF). The alveolar phase III slopes (SIII) from the double-tracer gas (DTG) mixture sulfur hexafluoride (SF6) and helium (He) may reflect VI in lung periphery. Objective : In this post-hoc analysis we calculated the DTG-SIII, the ratio of SF6-SIII over He-SIII, to assess treatment response from DA and HS. Methods : In 2 cross-over randomized controlled trials, 17 and 18 school-aged children with CF received 4 weeks DA or HS and placebo. Children performed MBW using 4% SF6 and 4% He. The DTG-SIII was calculated across 25 breaths of the washout. Average and variability (coefficient of variation) of DTG-SIII were the primary outcomes and expressed as change (mean difference [95%CI]) in percent from baseline. Results : In the DA study, both the average and variability of DTG-SIII decreased significantly by -63.1% (-121.6%; -20.0%) and -95.3% (-195.4%; -29.1%). In contrast average and variability of DTG-SIII did not change significantly in the HS study: -9.6% (-40.1%; 14.2%) and -26.6% (-70.8%; 6.2%). Individual changes exceeding the test variability of average and variability of DTG-SIII were observed in 13/17 and 15/17 children (DA study), and in 4/17 and 8/17 (HS study). Conclusion : Treatment with DA may systematically change VI possibly arising in lung periphery. Lung function response exceeding the test variability was more frequently observed in children using DA compared to HS.
