Abstract
When the International Society selected Durban South Africa as host of the XIII World Conference several years ago few anticipated the portentous controversial and potentially groundbreaking consequences of that choice. The selection of the first developing country venue for an international conference was welcome and long overdue. Despite the official themes of the last two conferences - World One Hope (Vancouver) and Bridging the Gap (Geneva) -- it has been clear to all observers that the chasm between the rich and poor nations with respect to HIV control and treatment has been growing briskly. Placing the international conference in a developing country therefore was both an act of solidarity and an effort to bring the science and advocacy of HIV to where it is most needed. Hopes for a breakthrough conference therefore were high as planning for the conference began. The attention of the world however was even more drawn to Durban by the confluence of three major forces that were little anticipated when the venue was chosen: The alarming and dramatic intensification of the HIV epidemic in southern Africa. The incomprehensible and dangerous flirtation of South African President Thabo Mbeki with the so-called AIDS Dissidents and his appointment of a presidential panel to investigate the causes of and responses to AIDS. The growing demand for access to highly active antiretroviral therapy for the millions of people with HIV living in the poorest countries of the world. (authors)
