Abstract
Las adolescentes presentan las tasas mas altas de infeccion por el virus del papiloma humano (VPH), lo que se atribuye a la combinacion de conductas sexuales de riesgo con la susceptibilidad biologica. La mayor parte de las infecciones y las anomalias citologicas asociadas son transitorias, de modo que tanto la infeccion como las lesiones desaparecen de manera espontanea. Este hallazgo ha impulsado nuevos enfoques terapeuticos, como la observacion de las adolescentes con anomalias citologicas. Las pacientes adolescentes con celulas epidermoides atipicas de significado incierto o lesiones intraepiteliales epidermoides de grado bajo han de someterse a estudios citologicos de seguimiento a intervalos de 1 ano hasta un maximo de 2 anos, momento en el que se realizara una colposcopia. El tratamiento de las portadoras de una neoplasia intraepitelial cervicouterina (NIC) 1 confirmada por biopsia es semejante al anterior, de modo que se realizan estudios citologicos anuales de forma indefinida o bien hasta la aparicion de lesiones intraepiteliales epidermoides de grado alto o NIC 2 o 3. El tratamiento de las adolescentes con NIC 2 o 3 que observan las prescripciones medicas puede basarse en la realizacion de estudios citologicos y colposcopicos cada 6 meses hasta un periodo maximo de 2 anos.
