Abstract
ÖZET Amaç: Toplumda kazanılan alt solunum yolu infeksiyonları arasında en sık saptanan ve antibiyotiklere gittikçe artan oranda direnç kazanan mikroorganizmalar Streptococcus pneumoniae ve Haemophilus influenzae'dır.Bu çalışmada 2010-2012 yıllarına ait çocuk hastaların solunum yolu örnekleri incelenmiştir.İncelenen örneklerde izole edilen mikroorganizmalar ve bu mikroorganizmalara karşı gelişen antibiyotik direnç durumlarının belirlenmesi amaçlanmıştır.Yöntemler: Ocak 2010-Ocak 2012 tarihleri arasında Cerrahapaşa Tıp Fakültesi Mikrobiyoloji Laboratuvarına gönderilen toplam 1834 solunum yolu örneği incelenmiştir.Bakterilerin identifikasyonu klasik yöntemlerle yapılmıştır.Antibiyotik duyarlılıkları Clinical and Laboratory Standards Institute (CLSI) kriterlerine göre, Mueller-Hinton agar kullanarak Kirby-Bauer disk difüzyon yöntemiyle çalışılmıştır.Bulgular: Bu çalışmada toplam 501 (%27,3) solunum örneğinde patojen mikroorganizma üremiştir.Üreyen bu mikroorganizmaların 353'ü polikliniğe başvuran hastalardan, 148'i ise serviste yatmakta olan hastalardan izole edilmiştir.Haemophilus influenzae, Streptococcus pneumoniae ve Pseudomonas aeruginosa en sık izole edilen bakteriler olmuştur.S. pneumoniae suşlarında %2,5 orta düzey ve %5 yüksek düzey penisilin direnci saptanmıştır.Haemophilus cinsi bakterilerde %2 oranında beta-laktamaz pozitifliği saptanmıştır.Sonuç: S. pneumoniae'nın etken olduğu solunum yolu enfeksiyonlarının ampirik tedavisinde penisilinlerin kullanılmasının uygun olacaktır, fakat penisiline dirençli pnömokokların bulunabileceği düşünülerek duyarlılık testlerinin mutlaka yapılması gerekmektedir.Gelişen solunum yolu infeksiyonlarında etkenin saptanması ve antibiyotik direnç oranlarının periyodik olarak belirlenmesi tedavinin yönlendirilmesi açısından gerekli olduğu düşünülmektedir.
