The Effect of Body Mass Index on Developments Patellar Tendinosis After Intramedullary Nailing of a Tibial Shaft Fracture

Abstract
Amaç: Tibia diafiz kırıklarının tedavisinde kullanılan intramedüller çivileme (İMÇ) sonrası patellar tendinit dizönü ağrılarının önde gelen nedenlerindendir.Çalışmamızda amaç opere İMÇ hastalarında kaynama sonrası hastalarda sağlam ve opere taraf arasındaki patellar tendon değişikliklerini MR görüntüleme ile karşılaştırmaktır.Gereç ve Yöntem: Çalışmamız Temmuz 2010 ile Eylül 2011 tarihleri arasında kliniğimizde tibia kırığı nedeniyle transpatellar yolla İMÇ uygulanmış dizönü ağrısı olan 12 hasta üzerinde yapıldı.Kırık kaynamasını takiben yapılan sağlam ve kırık taraf karşılaştırmalı MR görüntülemesi ortalama 19.9 ayda (14-28) yapıldı.Ölçümler belirlenen sabit anatomik bölgelerde yapıldı.Sagital plan ölçümleri patellar tendon 3 parça bölünüp, üst-orta-alt bölümler olarak değerlendirildi.Bulgular: Kırık taraf sagital proksimal, orta, distal ölçümleri sağlam tarafa göre istatistiksel olarak anlamlı düzeyde yüksek bulundu (p<0.01).Kırık taraf aksiyel ölçümleri de sağlam tarafa göre istatistiksel olarak anlamlı düzeyde yüksekti (p<0.05).Opere taraftaki patellar tendon kalınlaşmalarının, tendon distaline doğru arttığı gözlendi.Sonuç: Tibia diafiz kırığı nedeniyle İMÇ yapılan hastalarda dizönü ağrısı en sık gözlenen komplikasyonlardan biridir.Dizönü ağrı nedenlerinden biriside patellar tendondaki kalınlaşmadır.Bunu önlemek için nazik doku açılması ve oyma sırasında doku koruyucularının kullanılmasına özen gösterilmelidir.
