Abstract
To the Editor—We agree with Gadsby et al that it would be a tremendous boon if one could determine the pathogen in pneumonia using a multiplex molecular test on sputum [1]. However, we have concerns about the validity of their study. “Sputum” is sometimes saliva. The authors used the assumption that the presence of the human gene product GAPDH is proof that a specimen came from the lower respiratory tract and therefore is suitable for analysis. While the detection of GAPDH may indicate the quality of nucleic acid extraction and human cell content, one would presume that both epithelial cells and neutrophils would contain this gene. Therefore, the assumption that quantification of the GAPDH gene can act as a marker for sputum quality is flawed. This would explain why the authors reached the unexpected conclusion that all of the 323 specimens submitted for the study were adequate samples from the lower respiratory tract.
