Abstract
Opis przypadku/Case report StreszczenieNadciśnienie wrotne jest rzadkim problemem zdrowotnym u osób w wieku rozwojowym, a jedną z jego przyczyn może być zakrzepica układu wrotnego.Pacjentka 17-letnia została przyjęta do kliniki z powodu objawów krwawienia z przewodu pokarmowego.W badaniu endoskopowym górnego odcinka przewodu pokarmowego uwidoczniono żylaki przełyku, świadczące o rozwoju nadciśnienia wrotnego.W badaniach obrazowych stwierdzono zakrzepicę oraz przebudowę jamistą żyły wrotnej.Na podstawie całości obrazu klinicznego nie można wykluczyć, że nadciśnienie wrotne oraz jamiste przekształcenie żyły wrotnej były konsekwencją zakrzepicy żyły wrotnej jako powikłania splenektomii wykonanej ze wskazań hematologicznych w 10. roku życia (małopłytkowość samoistna niepoddająca się standardowemu leczeniu).Przypadek przedstawiono w celu zwrócenia uwagi, że pacjenci pediatryczni po splenektomii powinni być pod stałą opieką specjalistyczną pomimo braku jakichkolwiek objawów klinicznych.W tej grupie chorych należy koniecznie okresowo wykonywać badania obrazowe w celu monitorowania rozwoju powikłań pooperacyjnych w postaci zakrzepicy żylnej układu wrotnego i rozwoju nadciśnienia wrotnego.
