Abstract
Zielsetzung: Untersuchung von Perfusionsstörungen und fokalen Infarkten des Nierengewebes im Rahmen der akuten Nierentransplantatabstoßung im Mausmodell mittels Arterial Spin Labeling (ASL) und morphologischen T2 TSE-Sequenzen. Material und Methodik: Eine akute Abstoßung wurde durch allogene Nierentransplantation (NTx) von C57Bl/6-Nieren auf Balb/c-Mäuse mit kalten und warmen Ischämiezeiten von 60 bzw. 30 min induziert (n = 11). Kontrolltiere nach isogener NTx (C57Bl/6-Niere auf C57Bl/6-Maus) mit entsprechenden Ischämiezeiten entwickelten einen Ischämie-Reperfusionsschaden und keine Abstoßung. Am 7T-MRT wurden 1 und 6 Tage nach NTx T2 TSE- und FAIR ASL-Sequenzen akquiriert. Anhand der T2-Bilder und der errechneten Perfusionskarten wurden Infarkte des Nierengewebes und mittlere Nierenperfusion analysiert. Histologische Untersuchungen umfassten die Beurteilung der Abstoßung (Banff) und das Vorliegen von Infarkten. Werte sind als Mittelwert ± SEM angegeben. Ergebnisse: Ein Tier nach allogener und zwei nach isogener NTx starben aufgrund chirurgischer Komplikationen. Die Histologie zeigte eine akute T-Zell-Abstoßung (Banff IIB/III) nach allogener NTx und war nach isogener NTx unauffällig. Nur nach allogener NTx waren fokale Infarkte und einzelne Totalinfarkte im MRT nachweisbar, was mittels Histologie bestätigt wurde. Die renale Perfusion war bei beiden NTx-Gruppen geringer als bei gesunden C57Bl6-Tieren. Nach allogener NTx sank die Perfusion von 282 ± 41 ml/(min*100 ml) am Tag 1 auf 80 ± 13 ml/(min*100 ml) am Tag 6 (p < 0,01). Nach isogener NTx blieb die Perfusion stabil. Am Tag 6 nach NTx war die renale Perfusion nach allogener NTx signifikant niedriger als nach isogener (80 ± 13 vs. 260 ± 33 ml/(min*100 ml), p < 0,001). Schlussfolgerungen: Die akute NTx-Abstoßung ist assoziiert mit einer stärkeren Perfusionseinschränkung als der alleinige Ischämie-Reperfusionsschaden. Nur in der Gruppe der NTx-Abstoßung waren fokale Niereninfarkte nachweisbar, so dass eine Assoziation mit den immunologischen Prozessen im Rahmen der Abstoßung zu vermuten ist.
