Abstract
AMAÇ: Çalışmamızda inme hastalarında iki cinsiyet arasındaki vasküler risk faktörlerinin ve inme alt tiplerinin karşılaştırılması amaçlanmıştır.YÖNTEMLER: İnme Polikliniği’mizde 1996-2011 yılları arasında takip edilen toplam 2798 hastanın tıbbi dosyası gözden geçirildi. Klinik değerlendirme bilgileri, fizik ve nörolojik muayene ve nörogörüntüleme tetkiklerine dayanarak, inme alt tipi sınıflaması yapıldı ve risk faktörleri incelendi. Veriler SPSS 11.5 veri tabanında toplandı ve istatistiksel hesaplamalar için Pearson ki-kare ve Mann-Whitney U testi kullanıldı.BULGULAR: Toplam 2798 hastanın 2564’ü (%91,6) iskemik tipte inme tanısı almıştı, 234’ü ise (%8,4) hemorajik tipte inme tanısı ile takip edilmekte idi. Ortalama takip süresi 50.2+42.7 ay idi. Hastaların 1289’u kadın (%46), 1509’u erkek (%54) cinsiyette idi. İnme alt tiplerine bakıldığında, nedeni bilinmeyen inme alt tipi her iki cinsiyette de en büyük grubu oluşturmakta idi. Bunu takiben, erkeklerde en sık aterotrombotik tipte inme izlenirken, kadınlarda kardiyoembolik inme görülmekteydi. Koroner kalp hastalığı erkeklerde, atriyal fibrilasyon ve diğer ritim bozuklukları ise kadınlarda anlamlı derecede daha yüksek oranda izlendi. Sigara ve alkol kullanımı ise erkeklerde oldukça belirgin bir şekilde daha fazla idi.SONUÇ: Çalışmamızda, inme alt tipleri ve vasküler risk faktörlerinin cinsiyete göre farklılıklar gösterdiği saptanmıştır. Bu farklılıkların ve bunu etkileyen faktörlerin iyi bilinmesi, kadınların inme risk faktörlerinden, bu konudaki farkındalıklarına, hormonal değişiklik gösteren gebelik ve menopoz gibi dönemlerinden, depresyona kadar inme ile ilgili belirsizliklerin ortadan kalkması, daha iyi primer ve sekonder koruma tedbirleri almaya yardımcı olacaktır.
