Abstract
【背景】路上生活者数の増加に伴い救急車搬送される路上生活者の病院受入困難事例が増加し社会問題となっている。しかし,本邦で本問題を救急医療の観点から検討した研究は少なく,その実態は不明である。【目的】路上生活者の救急要請から診療終了までの状況を調査し,救急要請の応需と診療の遂行に支障を来す因子を解明する。【方法】救急隊搬送記録および病院データを用いて,路上生活者および非路上生活者の救急診療を後方視的に比較・検討した(χ2検定,p<0.01を有意差ありとした)。【結果】1.路上生活者は非路上生活者と比べ,救急車利用率(54.5% vs. 19.6%)が高いにもかかわらず,入院率(23.5% vs. 30.2%)は低かった(ともにp<0.001)。2.路上生活者は夜間に比べ,日中に救急受診することが多かった。3.救急受診をした路上生活者を救急受診時に路上生活者であると判明していた群(応需時ホームレス判明群)と判明していない群(応需時不明群)に分けると,後者は入院率(13.9% vs. 63.7%)が高く(p<0.001),病院滞在期間も長かった。4.路上生活者の入院時診断として消化器疾患が23.3%と最も多かった。遷延性意識障害があると入院期間が長かった。【結論】救急要請の応需および診療に支障を来す因子として,応需時不明群であること,遷延性意識障害があることが挙げられた。理由は,身元の特定,生活保護認定の取得,退院先の決定に時間を要することなど,行政手続き上や,治療は不要となっても患者の状態に合致した生活環境を提供できないことであった。路上生活者は救急車への依存度が高く,重症化すると入院期間も長くなることから,普段受診できる医療体制が必要であると ともに,救急外来診療においては,帰去時に支援するための積極的な福祉の介入が必要である。
