Artykuł poglądowy/Review paper Wstęp Przecieki okołozastawkowe (ang.paravalvular leaks -PVL) są powikłaniem po zabiegach implantacji sztucznych zastawek serca, zarówno metodą chirurgiczną, jak i przezcewnikową.Autorzy artykułu skoncentrują się na przeciekach okołozastawkowych u chorych po chirurgicznej wymianie zastawki serca.Po chirurgicznej wymianie zastawki PVL mogą być wynikiem powikłania infekcyjnego zapalenia wsierdzia, poluzowania lub wyprucia fragmentu szwu chirurgicznego, mogą też powstać wskutek znacznych zmian typu włóknistego i kalcyfikacji w natywnym pierścieniu zastawkowym utrudniających prawidłowy kontakt między nim a pierścieniem sztucznej zastawki.Częstość występowania wszystkich PVL, w tym również niewielkich, ocenianych w przezprzełykowym badaniu echokardiograficznym, szacuje się na kilkanaście procent u osób po implantacji zastawki aortalnej (ang.Według danych z literatury kliniczne objawy PVL (niewydolność serca, hemoliza i zwiększone ryzyko wystąpienia infekcyjnego zapalenia wsierdzia) występują u ok.1-5% pacjentów po AVR lub MVR [4], przy czym częściej jawne klinicznie są przecieki wokół sztucznej zastawki mitralnej [5].
