Abstract
Subclinical hypothyroidism, defined as an elevation of plasma thyroid stimulating hormone in the presence of levels of thyroid hormone within the normal range, may affect more than 10% of some sectors of the population (eg, women older than 50 years). By definition, subclinical hypothyroidism is associated with few if any symptoms, but studies with ever more sensitive tools demonstrate detectable abnormalities in many systems, increasing pressure to treat these people. This article discusses responses of the body to a failing thyroid gland, abnormalities associated with this level of insufficiency, and guidelines for identifying patients who would benefit from treatment.
