Abstract
Die Diagnose einer Interstitiellen Lungenerkrankung (ILD) erfordert eine interdisziplinäre Zusammenarbeit von Klinikern, Radiologen und Pathologen. Bei einem Teil der Patienten werden zur Klärung der Diagnose Lungenbiopsien notwendig. Zur Probengewinnung stehen die chirurgische Lungenbiopsie (Bx), die Zangenbiopsie (ZB) und die Kryobiopsie (KB) zur Verfügung. Diese prospektive, randomisierte Studie will klären, ob die KB einen höheren Beitrag zur Diagnosestellung als die ZB bei Patienten mit V.a. ILD liefert. 33 Patienten mit klinischem und radiologischem Verdacht auf eine ILD wurden in derselben Untersuchung mit flexiblem Bonchoskop transbronchiale KB und ZB entnommen. Die pathologische Diagnose erfolgte verblindet. Die Präparate wurden morphometrisch auf die Gesamtfläche, die relativen und absoluten Anteile von Bronchus- und Alveolarkompartimente analysiert. Die Diagnosestellung erfolgte in einer interdisziplinären Konferenz unter Berücksichtigung des HR-CT. Das Diagnosespektrum umfasst Sarkoidose, CTD/UIP und IPF/UIP, NSIP, RB-ILD, Pneumokoniose, EAA und unspezifische Veränderungen. Entnommen wurden insgesamt 205 Proben, davon 108 ZB und 97 KB (2 – 5 Biopsien pro Patient und Technik). Im Vergleich zur ZB sind KB signifikant größer (1,3 mm 2 vs. 6,62 mm 2 ). Das relative Alveolarkompartiment war in den KB signifikant größer (85,1% vs. 33,8%), die Bronchialfläche hingegen in den ZB (66,2% vs. 14,9%). Eine ILD konnte verlässlich bei 26 Patienten durch die KB diagnostiziert werden (79%). Beide Techniken zeigten keine signifikanten Unterschiede in der Blutungshäufigkeit. Es traten 2 Pneumothoraces auf. Die Diagnose einer ILD wird durch die KB signifikant erleichtert, so dass in einer Vielzahl der Fälle auf eine Bx verzichtet werden kann. Die KB war in dieser Studie eine sichere Methode und könnte als eine alternative Technik im diagnostischen Algorithmus der ILD Bedeutung erlangen.
