Abstract
Aims: Der Barrett-Ösophagus (BÖ) wird als eine präkanzeröse Läsion des distalen Ösophagus angesehen, die durch den Ersatz von Plattenepithel durch Zylinderepithel charakterisiert ist. Entsprechend der Montreal-Klassifikation wird der BÖ in einen Typ mit gastraler Metaplasie und einen Typ mit intestinaler Metaplasie unterteilt. Die sogenannten „SRY-related HMG-box“ Proteine (SOX) stellen hoch konservierte Transkriptionsfaktoren dar, die Schlüsselfunktionen in der Entwicklung von Geweben und Organen einnehmen. Ziele: Untersuchung der Genexpressionsprofile von drei verschiedenen SOX-Proteinen bei Patienten mit gastralem- und intestinalem BÖ. Methodik: 27 Patienten (8 Frauen 19Männer) mit einem BÖ (16 mit gastraler Metaplasie; 11 mit intestinaler Metaplasie) wurden untersucht. Die Genexpression von SOX-2, SOX-14 und SOX-17 wurden mittels quantitativer RT-PCR analysiert und statistisch in Bezug zu den endoskopischen und histopathologischen Befunden ausgewertet. Ergebnisse: SOX-2 wurde ubiquitär in allen Proben der gastralen- und intestinalen Metaplasie nachgewiesen. Die Transkriptmengen waren dabei in der gastralen Metaplasie um das 2,8 fache höher im Vergleich zu der intestinalen Metaplasie (p=0,002). Im Unterschied zum SOX-2 wurden SOX-14 und SOX-17 unterschiedlich exprimiert. Für beide Gene konnten in der intestinalen Metaplasie höhere Expressionsraten nachgewiesen werden (77% vs. 50%; p<0,05). SOX-14-mRNA Gehalte waren signifikant höher in der intestinalen als gastraler Metaplasie (Median: 0,07 vs. 0; p=0,015), wohingegen sich die SOX-17 Transkriptgehalte nicht zwischen beiden Metaplasieformen unterschieden. Schlussfolgerung: Der gastrale und intestinale Typ eines BÖ unterscheidet sich bezüglich der SOX-2, -14 und -17 Genexpression, was eine funktionelle Bedeutung dieser Transkriptionsfaktoren für die Entstehung einer Barrett-Metaplasie nahelegt.
