Epilepsian kliiniset piirteet ja kuolleisuus: systemaattinen kirjallisuuskatsaus ja meta-analyysi

Abstract
Epilepsiaan on liittynyt useissa tutkimuksissa ylikuolleisuus, mitä ovat selittäneet epilepsian taustalla olevat aivosairaudet, lisääntynyt itsemurhakuolleisuus sekä epileptisten kohtausten huonoon hoitotasapainoon liittyvät äkkikuolemat. Teimme 15 sähköistä tietokantaa käsittäneen systemaattisen kirjallisuushaun PRISMA ja MOOSE -suositusten mukaisesti. Etsimme kohorttitutkimuksia, joissa epilepsiaa sairastavien kuolleisuutta on verrattu muun väestön kuolleisuuteen. Löysimme 38 sisäänottokriteerit täyttänyttä epilepsiakohorttia. Epilepsiaan liittyvä kuoleman suhteellinen riski 165 879 potilasta käsittäneessä DerSimonian-Lairdin random effects meta-analyysissä oli 3.33 (95% luottamusväli 2.83 – 3.92; Higginsin I2–statistiikka 99%). Meta-regressioanalyysissä 42% (R2) riskiarvojen vaihtelusta selittyi eroilla kohorttien ikärakenteissa. Erot tutkimusten välisissä Newcastle-Ottawa Scale- laatupisteissä eivät olleet yhteydessä riskiarvioon (RR 1.00; 0.86 – 1.16). Eniten ylikuolleisuutta oli potilailla, joilla epilepsian etiologiana oli synnynnäinen tai kehityksellinen aivovaurio (10.3, 4.03 – 26.2; I2 70%), symptomaattinen (4.73, 3.27 – 6.83; I2 95%) tai kryptogeeninen etiologia (1.75, 1.20 – 2.54; I2 58%), sekä potilailla jotka eivät saavuttaneet kohtauksettomuutta (4.69, 1.41 – 15.60; I2 97%). Idiopaattista epilepsiaa sairastavilla kuolleisuus ei ollut merkitsevästi suurentunut (1.29, 0.75 – 2.20; I2 36%) eikä myöskään niillä jotka saavuttivat remission (0.97, 0.73 – 1.30; I2 0%). Kohtaustyyppien tai epilepsiaoireyhtymien osuutta kuolleisuuteen ei ollut mahdollista selvittää meta-analyysillä. Epilepsiapotilaiden ylikuolleisuus liittyi selvimmin epilepsian taustalla osoitettavissa oleviin rakenteellisiin aivomuutoksiin ja huonoon hoitotasapainoon. Hoitotasapainon osuutta epilepsiaa sairastavien kuolleisuuteen tulisi tulevissa tutkimuksissa arvioida epilepsian etiologian suhteen ositetuissa analyyseissä, sillä aihetta on tähän mennessä arvioitu vain yhdessä tutkimuksessa.
