Abstract
熱射病に起因した急性肝不全に対して生体肝移植を施行した症例と人工肝補助療法を施行した症例をそれぞれ経験した。症例1は16歳の男性で運動中に卒倒した。前医入院後，一時的に意識状態が改善したが再び昏睡状態となり第4病日に急性肝不全の診断で人工肝補助療法を開始し同日，生体肝移植を施行した。術後2日目に意識状態の改善を得た。症例2は15歳の男性で運動中に卒倒し，その後熱射病による急性肝不全の診断で人工肝補助療法を開始した。昏睡状態が遷延したが第14病日に意識状態の改善を得た。両者とも神経学的後遺症なく軽快退院することができた。熱射病による急性肝不全は，播種性血管内凝固症候群や熱射病による脳症の合併により肝移植適応を含めた肝不全の評価が困難となる。本病態に対しては人工肝補助療法を行うことで肝性脳症による不可逆的脳障害の回避や肝性昏睡からの覚醒効果に期待し，その間に致死的合併症を制御しながら肝再生あるいは肝移植につなげていく治療戦略が妥当であると考えられる。
