目的: 開眼状態における脳波測定法を用いた前頭葉脳波スペクトル分析のうつ状態像の判定における有効性について検討した. 対象と方法: うつ状態群22名ならびに対照群21名に対して開眼状態での脳波測定による脳波スペクトル分析を行った. 脳波はFp1, Fp2に相当する位置より導出した. またSelf-rating Depression Scale, Social Adaptation Self-evaluation Scale, Gotow Alexithymia Questionnaireの質問紙調査を行った. 結果: 脳波スペクトル分析において, S波のパワースペクトル値は, うつ状態群で有意な増加を認めた. 脳波の各成分帯域の出現頻度は, うつ状態群でのα成分帯域の有意な低下およびθ成分帯域の有意な増加を認めた. 各質問紙調査においてうつ状態群と対照群に有意差を認めた. 対照群とうつ状態群の設定は, 質問紙調査結果等から妥当と考えられた. 考察: うつ状態群は, 安静時脳波による先行研究と同様に活動時脳波のパワースペクトルが増大すると考えられた. 脳波成分の出現頻度は, 安静時脳波による先行研究の結果と異なるが, 活動時脳波における特徴を示していると考えられた. 近年うつ病の診断や治療効果の判定などについては, 精神科医による問診, また質問紙等をはじめとした評価尺度が多数存在するが, 生理的指標を用いた客観的検査法はいまだ開発途上にある. 脳波検査は, 頭皮電極で得られる脳の電気活動を時間的, 空間的に記録し, 脳の活動状況を客観的に評価するものであるが, 従来の脳波検査は, 電源雑音を遮蔽した専用の脳波計測室で行う必要があった. 近年, 遮蔽空間が不要で覚醒開眼生活行動下での測定が可能な小型脳波計が開発されたが, 今回の結果より, 開眼状態における脳波測定法を用いた前頭葉脳波スペクトル分析について, うつ状態診断補助としての利用可能性が示唆された. 本機器を使用した検査は, 使用に際して環境的制限が少ないこと, さらには被験者にとって非侵襲的であり負担が少ないことから, さらなる研究により利用可能性を検討すべきと考えられた.
