Abstract
Ziel: Die Evaluierung der Rolle thorako-amniotischer Shunt in grossen makrozystischen CCAMs (congenital cystic adenomatoid malformations) Methodik: Restrospektive Studie zum perinatologischen Verlauf von 11 Feten mit isolierten makrozystischer CCAMs nach thorako-amniotischem Shunt Ergebnis: Die Shunt-Implantaion erfolgte zu einem mittleren Gestationsalter von 24,6 (17–32) Schwangerschaftswochen. Bei allen Feten lag eine signifikante Verschiebung des Mediastinums aufgrund eines vorwiegend makrozystischen Tumors vor, bei sechs Hydrops fetalis und bei einem Polyhydramnion. In drei Fällen war die Läsion unmittelbar vor dem Eingriff rapide gewachsen und in einem Fall lag bereits bei Diagnosestellung eine sehr grosse Läsion vor (CVR (CAM volume ratio)=3,2). Nach Einsatz eines einzelnen Shunts kam es in allen 11 Fällen zu einer Dekompression des gesamtem multizystischen Tumors und Rückbildung von Hydrops und Polyhydramnion. Ein schwer hydropischer Fet in der 17. Schwangerschaftswoche verstarb einen Tag nach dem Eingriff, alle anderen Feten uberlebten. Ein Shunt dislozierte 2 Wochen nach dem Eingriff ohne dass es zu einer erneuten Grössenzunahme des Tumors kam. Keine Patientin hatte einen vorzeitigen Blasensprung oder andere mütterliche Komplikationen in Zusammenhang mit dem Eingriff. Alle Patientinnen entbanden vaginal mit einem durchschnittlichen Gestationsalter von 38,1 SSW. Bei allen Neugeborenen wurde postnatal eine Lobektomie durchgefuehrt – in 10 Fällen in den ersten beiden Lebenswochen, in einem 3,5 Monaten nach Geburt. Die Diagnose CCAM wurde in allen 11 Fällen pathologisch bestätigt. Schlussfolgerung: Feten mit grossen makrozystischen Lungenläsionen entwickeln haufig Hydrops fetalis, was mit einer Mortalität von fast 100 Prozent verbunden ist. Thorako-amniotische Shunts bieten eine exzellente Behandlungsmoglichkeit mit hoher Erfolgs- und niedriger Komplikationsrate. Daher sollten Shunts bei sehr grossen CCAMs auch vor Entwicklung von Hydrops fetalis in Erwägung gezogen werden. CCAM - fetal lung lesion - fetal thoraco-amniotic shunt
