Abstract
症例は生後8か月の女児。哺乳不良と体重増加不良を主訴に当科を受診した。初診時，低ナトリウム血症，高カリウム血症，高レニン血症及び高アルドステロン血症を認めた。画像検査で右重複腎盂尿管を認め，腎尿路奇形に伴う続発性偽性低アルドステロン症(secPHA)と診断した。輸液治療と塩化ナトリウムの補充で神経学的後遺症なく軽快した。secPHAには心停止・痙攣・意識障害といった重篤な経過をたどる例も存在するが，適切な治療を行えば予後は良好である。低ナトリウム血症，高カリウム血症を呈する乳児については，本疾患を鑑別疾患の一つとして念頭に置くべきである。尿細管の未熟性に尿路奇形・尿路感染症が合併することで，ミネラロコルチコイド受容体や上皮性Naチャネルの機能低下が生じ，secPHAを発症すると考えられているが，分子生物学的な背景を含めて病態の詳細は不明であり，病態解明には更なる基礎研究の進歩が望まれる。
