Abstract

症例は生後8か月の女児。哺乳不良と体重増加不良を主訴に当科を受診した。初診時,低ナトリウム血症,高カリウム血症,高レニン血症及び高アルドステロン血症を認めた。画像検査で右重複腎盂尿管を認め,腎尿路奇形に伴う続発性偽性低アルドステロン症(secPHA)と診断した。輸液治療と塩化ナトリウムの補充で神経学的後遺症なく軽快した。secPHAには心停止・痙攣・意識障害といった重篤な経過をたどる例も存在するが,適切な治療を行えば予後は良好である。低ナトリウム血症,高カリウム血症を呈する乳児については,本疾患を鑑別疾患の一つとして念頭に置くべきである。尿細管の未熟性に尿路奇形・尿路感染症が合併することで,ミネラロコルチコイド受容体や上皮性Naチャネルの機能低下が生じ,secPHAを発症すると考えられているが,分子生物学的な背景を含めて病態の詳細は不明であり,病態解明には更なる基礎研究の進歩が望まれる。
