Abstract
Erwachsene mit angeborenen Herzfehlern sind eine neue, rasch wachsende Population mit speziellen Herausforderungen für die Ärzte und das Gesundheitswesen. Ihre kardiale Anatomie und Pathophysiologie sind häufig komplex und erfordern spezielle Kenntnisse und Erfahrungen, die am besten in einem Kompetenzzentrum für Erwachsene mit angeborenen Herzfehlern konzentriert sind und den Patienten zur Verfügung stehen. Rhythmusstörungen sind neben Herzinsuffizienz die häufigste Spätkomplikation. Auch sog. „harmlose“ Rhythmusstörungen können bei dieser neuen Generation von Patienten lebensbedrohlich sein. Etablierte Behandlungsprinzipien für Patienten mit erworbenen Herzerkrankungen lassen sich nicht auf Erwachsene mit angeborenen Herzfehlern übertragen: ein häufiges Missverständnis im klinischen Alltag. Patienten mit komplexen Herzfehlern und Rhythmusstörungen müssen an ein Kompetenzzentrum für Erwachsene mit angeborenen Herzfehlern zur umfassenden Abklärung und fachkompetenten Behandlung überwiesen werden. Die soeben publizierten, ersten Richtlinien zur Behandlung von Rhythmusstörungen bei Erwachsenen mit angeborenen Herzfehlern sind sehr umfassend und hilfreich.
