NVA237 einmal täglich bietet schnelle, klinisch bedeutsame und 24h anhaltende Bronchodilatation bei COPD-Patienten: die GLOW1 Studie

Abstract
Einleitung: NVA237 (Glycopyrroniumbromid) ist ein einmal täglich inhalativ anzuwendender langwirksamer Muskarinrezeptor-Antagonist in der Entwicklung zur Behandlung von COPD. Methode: Patienten mit moderater bis schwerer COPD wurden doppelblind auf NVA237 50µg, einmal täglich, oder Placebo randomisiert (2:1) und mithilfe eines widerstandsarmen, einzeldosierten Trockenpulverinhalators (Concept1) über 26 Wochen therapiert. Primärer Zielparameter war trough FEV 1 (Talspiegel des FEV 1 , Durchschnitt der Werte 23h 15min und 23h 45min nach Inhalation) gegenüber Placebo nach 12 Wochen. Ergebnisse: 822 Patienten wurden randomisiert; 80,5% schlossen die Studie ab (durchschnittliches Alter: 63,9 Jahre, mittlerer post-bronchodilatatorischer FEV 1 : 55% des vorhergesagten Normalwertes). Nach Woche 12 war ein statistisch signifikanter und klinisch relevanter Unterschied zwischen NVA237 und Placebo bezüglich trough FEV 1 festzustellen (108ml; p<0,001). Der trough FEV1 war außerdem signifikant höher an Tag 1 und Woche 26 (Behandlungsunterschied: 105ml und 113ml, jeweils p<0,001). Zu jedem Zeitpunkt an Tag 1, Woche 12 und Woche 26 zeigte NVA237 statistische (p<0,001) und klinisch bedeutsame Verbesserung des FEV 1 im Vergleich zu Placebo. NVA237 zeigte einen schnellen Wirkungseintritt mit erhöhtem FEV 1 von 93ml nach 5min und 144ml nach 15min gegenüber Placebo nach der ersten Dosis an Tag 1 (p<0,001). Die Häufigkeit unerwünschter Ereignisse (AEs) war vergleichbar zwischen NVA237 und Placebo (57,5% vs. 65,2%). Schwere unerwünschte Ereignisse (SAEs) wurden bei 7,5% der NVA237- und 9,0% der Placebo-Patienten berichtet. Schlussfolgerung: NVA237 50µg, einmal täglich, war sicher, gut verträglich und führte zu einer klinisch bedeutsamen Bronchodilatation mit schnellem Wirkeintritt, welche im Verlauf der gesamten Studie über 24h beibehalten wurde.
