Abstract
Die pulmonal-arterielle Hypertonie (PAH) manifestiert sich am stärksten in der Dyspnoe, Belastungsintoleranz und verminderten Lebensqualität. Die Mechanismen der Belastungsintoleranz bei PAH sind eindeutig multifaktoriell und umfassen kardiale, respiratorische und peripher-muskuläre Anomalien. Die Belastungskapazität ist somit ein Marker der integrierten Reaktion aller am O2-Transport und -Verbrauch beteiligten Systeme. Daher ist die objektive Messung der Belastungskapazität eine sinnvolle und relevante Maßnahme zur Beurteilung von PAH-Patienten, da sie eine Orientierung bezüglich der Schwere und des Verlaufs der Erkrankung sowie des Ansprechens auf die Therapie ermöglicht. Im Laufe der Jahre nahm die Messung der Belastungskapazität daher eine zentrale Rolle in der Patientenevaluation sowohl in klinischen Studien als auch in PAH-Kliniken ein. Ihre Responsivität auf pharmakologische Interventionen ermöglichte auch den Wirksamkeitsnachweis neuer PAH-spezifischer Therapien. Jedoch ist ihre Sensitivität zum Nachweis von Verbesserungen nach Therapiemaßnahmen jedoch in aktuellen Kombinationsstudien sowie bei Patienten mit weniger schwerer Erkrankung in Frage gestellt worden. Die Zuverlässigkeit von Belastungstests bei nicht idiopathischer PAH muss ebenfalls noch validiert werden. Diese Arbeit befasst sich mit der Physiopathologie der körperlichen Belastung und der klinischen Rele- vanz von Belastungstests bei PAH.
