Abstract
Objective To explore the association of baseline blood pressure level in 1992 and its 10 year change from 1992 to 2002 with carotid atherosclerosis in 1331 cohort subjects in Beijing Methods In 1992, 1985 subjects aged 35 64 years were recruited into a cohort on CVD risk factors in Beijing In 2002, the second survey on CVD risk factors and a B mode ultrasound examination of carotid artery were carried out in the same sample Complete data were available for 1331 subjects Results (1) In 2002, the prevalence rate of intimal medial thickening (IMT) and carotid plaque was 51 4% and 34 7%,respectively,in the sample of 45 74 years old (2) The prevalence rate of carotid plaque was 49 5% in the hypertensives, significantly higher than the rate in normaltensives (28 9%) ( P 0 001) The prevalence rates of IMT and carotid plaque increased with blood pressure The Odds Ratios (ORs) of IMT and carotid plaque were 1 8 and 5 6, respectively, for BP≥180/110 mm Hg, compared to the optimal blood pressure (3) The prevalence rate of carotid plaque and /or IMT was 50 6% in the subjects who had normal BP in both 1992 and 2002, whereas the rates were 63 6% in those who were hypertensive in 1992 but normaltensive in 2002 and 74 2% in those who were hypertensive in both 1992 and 2002, respectively Conclusion The carotid atherosclerosis was a prevalent disorder in the sample aged 45 to 74 years in Beijing The prevalence rates of IMT and carotid plaque increased with blood pressure The effect of BP≥180/110 mm Hg on carotid plaque was more significant than on IMT Effective control on hypertension will be helpful to prevent or postpone the occurrence of carotid atherosclerosis.
