Abstract
Objective: To analyze the clinical experience of malignant middle cerebral artery infarction(mMCAI).Methods: 31 patients with acute mMCAI within 12h after stroke were examine by head CT,TCD,and subtype-classified using TOAST.Initial clinical presentation was assessed by the Canadian Neurological Scale(CNS) and the Glasgow Coma Scale(GCS).19 patients were treated only with medical management,12 patients were performed with hemicraniectomy surgery after medical management but had progressive deteriorating.All survivors were followed up for 3 months and assessed according to Barthel Index(BI).Results: Mortality was 25%(3/12)in the decompressive group,and 42%(8/19) in the non-decompressive group,no significant differences in the mortality between 2 groups.The CNS and GCS at the 24h of death group was significantly lower than survival group.Conclusion: mMCAI were not one mode of course,hemicraniectomy could be reinforcements of conservative treatment.
