This chapter considers that increased major histo compatibility complex polymorphism may be advantageous in creating recognizable differences between fetal tissues and maternal immune cells, thus promoting improved fetal viability. It reviews the functional anatomy of the interfacing maternal and fetal tissues, and maternal immune recognition during pregnancy. The chapter examines how the fetus avoids the consequences of that immunity, and discusses the emerging evidence for a beneficial effect of maternal immunity on the fetus. The intimate nature of the placenta allows for sensitization of the maternal immune system. However, rejection of the fetus rarely occurs. The placenta thus may also behave as an immunologic barrier to maternal immunologic attack against her fetus. The development of the embryonic and maternal components, specifically the trophoblast and endometrial decidua, which make up the major sites of maternal-fetal interface, follows a well-defined similar pattern in humans and rodents.
