À partir d'une analyse de la validité scientifique et du niveau de mise en œuvre des recommandations des commissions publiques qui ont été nommées pour aviser le gouvernement du Québec sur l'avenir de son système de santé, nous discutons ici des obstacles qui entravent la mise en œuvre des réformes nécessaires pour l'avenir de ce système. Notre analyse montre que les principaux obstacles ne sont pas tant programmatiques (quoi faire), que politiques (comment faire). Nous nous appuyons ensuite sur la théorie néo-institutionaliste pour discuter des implications de ce constat.
