Abstract
After having spent a day at the beach, a 23-year-old man developed a slightly pruritic papular eruption on the anterior aspect of the neck and chest. The only constitutional symptoms were fever (temperature, 38.5°C), chills, and a mild headache. Cellulitis was suspected and he received treatment with parenteral cefazolin but did not experience improvement. Consultation with the infectious diseases and dermatology services at our institution was requested 6 days after the initial onset of the eruption because headache persisted and because the papular eruption had progressed to involve large areas of the face and torso. The patient's medical history was remarkable only for mild asthma and eczema; he specifically denied having a history of herpes labialis or sexually transmitted diseases. On examination, the patient appeared healthy, and his tympanic temperature was 38.1°C. The lesions were present from chin to chest (figure 1). In the center of the affected area, there was epidermal erosion with peripheral umbilicated papules and vesicles. Figure 2 shows a close-up of these lesions, which were 0.2–0.5 cm in diameter. The area was tender on palpation, and tender cervical adenopathy was present on the left. Lesions were not present on the posterior aspect of the neck, where only subacute eczema was present.
