PREVIEWThe symptoms of vascular dementia are often distinct from those of Alzheimer's disease. The memory deficits that define Alzheimer's disease are not always observed in the initial stages of vascular dementia, which is usually characterized by greater impairment of executive function. However, increasing evidence supports an involvement of the cholinergic system in vascular dementia similar to that seen in Alzheimer's disease. In this article, Dr Black reviews the pathogenesis and diagnosis of vascular dementia, risk factors for the disease, and current treatment approaches, including possible use of Cholinesterase inhibitors.
