Abstract
La mise sur le marche en France d'un medicament destine au sevrage tabagique est assortie de la mise en place par l'Afssaps d'un programme de surveillance justifie par le risque de survenue de crises convulsives et le risque potentiel d'abus ou de dependance. Le bupropion (BAN - British approved name) ou amfebutamone (DCI) deja commercialise sous le nom de Zyban® dans de nombreux pays pour cette indication, ainsi que comme antidepresseur sous le nom de marque Wellbutrin®, a fait l'objet de recherches comme cause possible de deces au Canada (4), en Grande-Bretagne (18) et en Allemagne (4). Tout comme celle d'autres antidepresseurs, la consommation de bupropion s'accompagne d'importants risques d'accidents cerebrovasculaires (convulsions 0,1 %). Afin de limiter le risque de crises convulsives, il est recommande de respecter les contre-indications (antecedents de troubles convulsifs, anorexie mentale ou boulimie, prise concomitante d'inhibiteurs de la monoamine oxydase) ; par ailleurs l'utilisation de ce medicament est deconseillee chez les patients qui ont souffert d'un traumatisme crânien, qui sont dependants a la cocaine ou aux opiaces ou qui viennent de reduire leur consommation d'alcool ou d'autres sedatifs.\rvoir nouvelles MT n°7, 2001 :\r1.
