Abstract
We read with interest the letter by Laszlo et al regarding their experience in the management of AF in a community hospital in Hungary. As highlighted, there is a difference in the prevalence of permanent AF in their experience and that of the European Heart Survey. Their success rate with antiarrhythmic medications was also different from that of the previously reported randomized clinical trials. These differences are probably due to differences in the patients being studied. Finally, they reported a high rate of patient anticoagulation, which could be due to an aggressive anticoagulation follow-up program and/or implementation of clinical guidelines. We agree that the management of AF is complex and challenging. Although we agree that it is necessary to individualize therapy for patients with AF, we also believe that the current ACC/AHA/ESC guidelines provide a good starting point that is evidence-based and suitable for the majority of patients with AF.
