Abstract
The reported survival outcomes on COG ACNS0122 study for NGGCT patients who achieved a complete or partial response to induction chemotherapy have been excellent (>90%) suggesting a role for reduction of therapy in a select group of patients. Stratum 1 of currently open COG trial, ACNS1123, is evaluating 3-year progression-free survival in patients with localized/bifocal CNS NGGCT who receive 3060Gy whole ventricular irradiation plus involved field boost instead of craniospinal irradiation following chemotherapy. Additionally, neuropsychological functioning will be assessed for all patients as a primary aim using mandatory enrollment on COG ALTE07C1 study. 186 eligible patients have been enrolled on the trial to date, of which 93 have been on NGGCT stratum (target: 248 total and 125 NGGCT). There are 24 females and 69 males with a median age 10.98 years (range: 368-21.63 years). Tumor location was pineal in 49, suprasellar in 34, ventricular in 5, and bifocal in 5 patients. Pineal tumors occurred primarily in males (66.7%) and suprasellar tumors in females (79.1%). 73% of patients had hCG□ elevation, 64% had AFP elevation, 50% had both, and 25% had elevation of AFP and/or hCG□ > 1000). There was no significant correlation between site of disease and sex with either hCG□ or AFP elevation. Grade 4 toxicities have been mainly related to myelosuppression and DI associated electrolyte imbalances with no unexpected severe adverse events or deaths attributable to therapy. Though early, this study shows tolerability of this combined chemotherapy and reduced dose radiation therapeutic strategy in subset of patients with CNS NGGCT.
