Abstract
Цель и задачи исследования провести оценку клинико-патогенетической значимости изменений адсорбционно-реологических свойств сыворотки крови (АРСК) у больных с разными формами течения подагрической нефропатии. Материал и методы. С помощью межфазной тензиореометрии крови обследованы 89 больных и 30 практически здоровых людей контрольной группы (преимущественно мужчин). Использовали компьютерные аппараты MPT2-Lauda, ADSA-Toronto и PAT2-Sinterface. Уролитиазный тип нефропатии диагностирован в 39 % случаев, латентный в 61 %. Снижение функции почек установлено у 54 % обследованных больных. Результаты и их обсуждение. Вариант подагрической нефропатии оказывает достоверное влияние на интегральные параметры мочи, определяя уровни протеинурии, лейкоцитурии, оксалатурии, урикурии, фибронектинурии, b2-микроглобулинурии и нитритурии, клиренс креатинина, мочевой кислоты, оксипуринола и кальция, а уролитиазный тип поражения почек проявляется большим по сравнению с латентным типом поверхностным натяжением сыворотки крови, что связано с параметрами функции почек, имеет прогностическую значимость, влияет на межфазную поверхностную активность и вязкоэластичные свойства мочи. Выводы. Изменения АРСК отражают характер течения подагрической нефропатии, отдельные показатели участвуют в патогенетических построениях заболевания и могут иметь прогностическую значимость.
