Abstract
Malgre leur rapide essor, les soins palliatifs restent d'acces limite, particulierement en raison du manque frequent de coherence dans leur planification. Deux programmes de soins palliatifs, l'un canadien, l'autre catalan, illustrent comment le developpement de soins palliatifs etroitement Integres aux reseaux de soins conventionnels et coordonnes entre eux ameliore significativement l'acces et la qualite des soins, sans couts financiers additionnels significatifs. Les etapes clefs de ces projets sont le recensement des besoins et des ressources existantes, la mise en oeuvre combinee de mesures generales (legislation, formation, financement approprie des prestations medicales a domicile, etc.) et specifiques (equipes mobiles et unites de lits) a tous les niveaux du reseau de soins et le suivi des prestations et de leur qualite. Des modeles similaires se developpent en Suisse romande.
