La question de la prise en charge communautaire de la santé au Québec est aujourd’hui principalement polarisée autour de celle du rôle des Centres locaux de services communautaires (CLSC) institués par la réforme des services de santé et des services sociaux en 1971. Le modèle des CLSC est la résultante de deux fonctions principales que doit gérer l’Etat québécois : une fonction économique de diminution draconienne des coûts pour laquelle le modèle du CLSC joue un rôle primordial car il propose des soins simples, globaux, préventifs, de base, etc., et une fonction sociale, celle d’exercer le contrôle politico-social des catégories sociales pauvres par une politique de participation intégrative. Toute l’évolution récente des CLSC est marquée par la tension que suscite la gestion simultanée de ces deux fonctions contradictoires sous plusieurs aspects. Le contexte de crise économique favorise la prédominance de la fonction économique, ce qui se traduit par une volonté gouvernementale croissante d’intégrer les CLSC dans le réseau des établissements socio-sanitaires, de les normaliser, au détriment de leur fonction d’innovation, de participation, de contestation, d’orientation communautaire, qui donne son caractère original au modèle théorique des CLSC. Aussi les CLSC sont-ils actuellement le lieu d’enjeux importants dans les luttes pour une transformation des pratiques de santé au Québec.
