Abstract
Aim: The aim of this study is to gain increased knowledge on the experience relatives endure when an older family member moves to a nursing home. This study seeks to understand and synthesize the relatives’ relational experiences. Background: The percentage of older adults that will require nursing home care will increase dramatically over the next years. It is expected that relatives can become more involved in the daily care of the family member who lives in a nursing home. This requires close collaboration with the institution. These factors necessitate the importance of gaining an understanding of the relatives’ experiences in relation to the nursing home. Institutionalisation of a family member can put a relatives own health at risk. Strong and sustainable relationships between relatives and the nursing home staff are thus important. Design: Metasynthesis and metaethnography. Theoretical perspective: Kirkevold`s view on relatives in nursing and Kasen`s theory on relations in nursing care. Methodology: Metasynthesis comprising extensive database and manual searches, which after Critical Appraisal Skills Programme analysis included 14 qualitative studies, where 315 informants shared experiences regarding a close relative moving to a nursing home. Findings: The included studies took place in Canada, America, Northern Ireland, Ireland, England, Scandinavia, Taiwan and Australia. Relatives can experience both lack of information and help from the health care system, and be perceived by staff as inspectors of care. They want to be regarded a resource and not be considered difficult. They express the need for collaboration with, and recognition from the nursing home staff. Family members experience the process of nursing home placement as a last phase, and can be left with feelings of guilt, loss, loneliness and remorse. They want to keep in touch with their relatives and their daily lives and pay back the care they themselves received. Relatives look upon themselves as the hub in the family, knowing that their relatives receive good care in the nursing homes makes them appreciative and gives them peace of mind. Clean localities and clean habitants, number of staff and quality of care as well as physical distance to the nursing home where other important issues. The metaphor “an emotional walk in a rocky landscape” binds the findings together. Conclusion: This study shows that changes in relationships are a central experience when a family member moves to a nursing home. This can be in relation to the family member in the institution, in relation to themselves and relationships with the rest of the family. It is thus of great importance to gain an understanding of how to meet and cooperate with relatives in a good way when a family member moves to a nursing home. Implications for practice: Findings in this study can be used to enhance clinical practice by creating an awareness of relatives’ experiences when a family member moves to a nursing home.
