La mise sur pied des centres locaux de services communautaires au début des années 1970 au Québec correspondait à une situation particulière de crise dont les composantes se sont profondément modifiées depuis. Afin de saisir les nouveaux enjeux auxquels sont dorénavant confrontés les CLSC, les auteurs se penchent sur leurs modalités d’organisation et de fonctionnement. Deux dimensions doivent à cet égard être abordées conjointement, à savoir les rapports de travail et les rapports de consommation. Au départ, la création des CLSC était caractérisée par une triple alliance entre direction, salariés et usagers. De plus, les CLSC exprimaient à cette époque autant un refus de la consommation dépendante qu’une critique des rapports de travail autoritaires. La remise en question du compromis initial s’est effectuée à travers la mise en place, sur le plan du travail, de rapports quasi fordistes (en remplacement des rapports socio-politiques) et, sur le plan de la consommation, de rapports catégoriels qui se sont substitués aux rapports socio-communautaires de consommation. L’avenir des CLSC se joue autour des deux dimensions.
