Dans cet article, on essaie de dépasser la confusion sémantique dans laquelle se trouve l’expression « autonomie » dans les discours bioéthiques d’aujourd’hui. Dans un premier temps, on présente les racines historiques de ce concept philosophique. Et, dans un second, on cherche à situer l’histoire de la même catégorie dans l’histoire de la littérature médicale et au sein des débats contemporains, en essayant d’en problématiser et critiquer la portée normative. Enfin, dans un troisième temps, on analyse les rapports qui peuvent se tisser entre le principe d’autonomie et le devoir d’accompagner les malades et mourants, et cela au sein des pratiques médicales.
