Le trouble conversif constituerait-il une pathologie à part ? Vécu comme une réalité par le patient, l’absence de signes objectifs à l’examen l’a longtemps faite considérer comme une « pathologie sans substrat ». Or, si la clinique de ce trouble est bien connue, les motivations précises qui le sous-tendent restent sujettes à l’interprétation et les mécanismes exacts qui la produisent sont encore mal compris. Ainsi, le fait de ne pas retrouver de lésion explicative dans le cerveau exclue-t-il la possibilité d’un dysfonctionnement au sein de ce même organe ? On qualifie bien souvent ces troubles de « maladie de l’imagination ou de la volonté », mais au final, on connaît peu de choses sur le fonctionnement cérébral du mouvement volontaire et des représentations mentales dans le trouble conversif. Alors, dans cette optique, quel pourrait être l’apport des moyens « modernes » d’investigation tels que l’imagerie fonctionnelle et la mise en évidence d’anomalies fonctionnelles dans le cerveau ne permettrait-il pas d’ouvrir de nouvelles perspectives théoriques pour ces patients ? On observe alors un démembrement nosologique de la clinique hystérique, dont la justification était d’ouvrir de nouvelles voies d’exploration et de compréhension du trouble. L’imagerie fonctionnelle a permis d’explorer des mécanismes plus complexes jusqu’à présent peu accessibles (émotions, états affectifs, volition, imagerie mentale, processus inconscients), et nous proposons dans cette communication de montrer comment, à l’aide de ces techniques, une pathologie considérée comme un diagnostic d’exclusion et de trouble sans substrat peut être abordée différemment du fait d’explications psychobiologiques « nouvelles ».
