La commission Romanow avait pour but de proposer un projet de réforme qui garantirait la viabilité à long terme d’un système de santé universellement accessible et financé par l’État. Cependant, de fortes pressions se font sentir en faveur d’un accroissement des dépenses de santé, et l’on doit établir des priorités dans le rationnement des soins de santé. Cet article définit les grandes lignes de la stratégie décrite par le rapport Romanow pour définir ces priorités et relie cette stratégie à des propositions visant à faire en sorte que l’évaluation des innovations médicales (telles que les technologies de la santé et les médicaments) soient fondées sur des données probantes. En s’appuyant sur des exemples tirés du domaine de l’évaluation des technologies de la santé, cet article montre pourquoi il est peu probable que cette approche donne lieu à une limitation effective de l’accès à ces ressources technologiques, à moins qu’elle ne s’accompagne d’un débat sur les valeurs publiques.
