Abstract
This article presents a new framework for documentation based on the Canadian Model of Occupational Performance and the World Health Organization's International Classification of Functioning, Disability and Health. It is the outcome of a review of occupational therapy clinical documentation in an older adult's rehabilitation setting in a New Zealand public hospital. The 'Subjective-Objective-Assessment-Plan' format was predominantly being used for note writing and focused on clients' physical skills in self-care tasks contributing to the loss of the unique nature and philosophies of occupational therapy. The new framework is now a service policy and being used by other occupational therapy services and has the potential to be used in all areas of practice.
