救急外来におけるJTAS導入による効果の検討(A study of the effect of introduction of JTAS in the emergency room)

Abstract
【目的】救急外来のwalk–in患者におけるJTAS(Japan triage and acuity scale)導入による効果について検討する。【対象・方法】2006年4月から2010年12月までの簡易トリアージを施行した4年9か月間(Term A)と,2011年1月から2015年9月までのJTASを導入した4年9か月間(Term B)において,救急外来で急変した症例および急性冠症候群疑い例における緊急心臓カテーテル(緊急心カテ)室入室時間を比較検討した。【結果】重症患者は,Term Aで653人,Term Bで626人だった。このうち,トリアージとして問題がある症例は,Term Aで1例,Term Bで1例あり,急変の頻度に有意差はなかった。また緊急心カテが必要な患者は,Term Aで182人,Term Bで167人いた。初回の心電図でST上昇ありの場合,受付から医師診察までの中央値はTerm Bで4.5分有意に短縮したが,受付から緊急心カテまでの中央値に有意差はなかった。初回の心電図でST上昇なしの場合,受付から医師診察までおよび受付から緊急心カテまでの中央値に,いずれも有意差はなかった。【結語】救急外来におけるJTAS導入による効果は,急変の頻度および緊急心カテまでの時間に関して,限定的なものであることが示唆された。
